Answer: D. New guidelines on vancomycin therapeutic monitoring for serious Staphylococcus aureus infection were issued by the American Society of Health-System Pharmacists, the Infectious Diseases Society of America, the Pediatric Infectious Diseases Society, and the Society of Infectious Diseases Pharmacists in April 2020. For patients with known or suspected severe S. aureus infection, the guidelines recommend using the 24-hour time-concentration curve (AUC) rather than the trough vancomycin level to guide adjustments in maintenance dosing to maximize clinical efficacy and minimize nephrotoxicity risk. AUC-guided dosing requires use of an AUC calculator and the assistance of a pharmacist.
